Required documents (ALL DOCUMENTS NEED TO BE TURNED IN AT THE SAME TIME):
- Volunteer Form (click for a PDF version)
- TB Test Confirmation – valid for 3 years from date of approval
- CDL and Proof of valid Insurance if you plan to drive during a field trip
TB Tests are no longer conducted through the school nurse or through the County of San Diego for those individuals with insurance: TB services are not provided for individuals with health insurance. Insured individuals should visit their doctor. TB testing may also be available at community health centers, walk-in clinics and pharmacies such as CVS and Rite Aid.
Volunteers must fill out a new volunteer form and submit the above documents every new school year. The school does not keep the previous forms on file for approval in subsequent school years.
The office is currently accepting Volunteer applications only on Wednesdays.
The deadline is Wednesday, November 11, 2015 by 4:30 p.m.
Volunteers should have their picture taken on picture day. If a picture is taken, a volunteer badge will be created and getting on campus will be easier for the volunteer. Without a volunteer badge, Volunteers may have to show their ID to come on campus or if they don’t have ID they may be refused campus entry.
